Kate is the Care Community Support Manager for Congleton and Holmes Chapel (CHOC) with Cheshire East Integrated Care Partnership.

The Care Community includes multiple partners and organisations with the aim of providing a range of integrated health and social care services within the community setting.

Kate’s role involves supporting projects and initiatives that improve the health and wellbeing of the local residents and to facilitate collaborative working between the numerous partners of the care community.

Kate has joined Holmes Chapel Partnership as an NHS partner representative and is excited to work with the partnership to promote health and wellbeing within Holmes Chapel.
